1,25 Dihydroxy Vitamin D is the active metabolite, but is not routinely used as a measure of Vitamin D deficiency as levels tend to be preserved even when 25 Vit D is low.

It is most useful in the investigation of hypercalcaemia in suspected granulomatous disease, where the PTH production is suppressed, or for monitoring patients on Calcitriol (1,25 Vitamin D) therapy.

  • Lithium heparin plasma is also an acceptable specimen.
  • EDTA plasma may be acceptable (under exceptional circumstances) but consult Biochemistry staff as plasma should be separated within 8 hours, if not immediately.
  • Grossly haemolysed or lipemic samples should be rejected.
