Ordering Information	Please refer to Molecular Genetics - Austin Pathology for specific testing details and request form for ordering, indicating the test required and the biopsy number and date of collection of the sample you wish to be tested. After this form has been completed, please forward it to the Anatomical Pathology laboratory via the fax number found on the request form.
Collection Instructions	At least 10% tumour nuclei 5μm sections (Both “Tumour” and the “Normal” Tissue is required) on uncoated slides – 5 sections for resection specimens OR 8 sections for small biopsies or resections with very focal tumour OR FFPE tissue block Copy of the histology or cytology report
