Ordering Information	Tests on Healthcare Worker: Hepatitis B surface antibody and serum stored. Tests on the Source: Hepatitis C antibody, Hepatitis B surface antigen, HIV antibody and serum stored. The requesting doctor must get consent from the patient prior to collecting blood for HIV testing. The requesting doctor must sign the request form before HIV testing is done.
